Aviation safety reports, made easier to study

NTSB Reports is an independent research and search tool for public aviation accident and incident records. It brings NTSB aviation reports, structured fields, docket files, extracted figures, photos, videos, and update history into one searchable interface.

The goal is practical safety education: make it faster to find relevant investigations, compare similar events, review factual records, and understand the evidence behind probable-cause and preliminary reports.

The investigative work and source records belong to the National Transportation Safety Board and the published investigation record. This site is not affiliated with or endorsed by the NTSB.

Full Narrative

On May 20, 1994, about 1040 central daylight time, a Grumman G-164B, N6816Q, collided with a dike during an aerial application swath. Subsequently, it nosed over during landing at Memphis, Tennessee. The airplane was operated by Thomas Munn, Jr. and Jim Harris, Jr. under 14 CFR Part 137, and visual flight rules. Visual meteorological conditions prevailed, and a flight plan was not filed. There were no injuries to the commercial pilot, and the airplane was substantially damaged. Origination of the flight was Wheatley, Arkansas, about 0800 on the same day. During the dike collision, the right main landing gear was separated from the airplane, and the left main gear was damaged. The pilot diverted to General Dewitt Spain Airport, at Memphis. During the landing slide on the hard surface runway, the airplane nosed over.
