After an approach to runway 5, and touched down at 85 knots, the airplane yawed right, exited the runway, the right main landing gear collapsed, and the airplane nosed over. Examination of the airplane revealed that a right main wheel brake had locked up, and the landing gear had collapsed. Inspection of the right main landing gear assembly and all associated components could not provide any determination as to what caused the main wheel brake to lockup. The brake assembly was broken down into its component parts and inspected. No evidence of malfunction could be detected. No contamination of the hydraulic fluid was evident.

On September 26, 2000, about 0635 eastern daylight time, a Douglas DC-3, N12907, nosed over during the landing roll at Douglas International Airport, Charlotte, North Carolina. Visual meteorological conditions prevailed at the time, and an instrument flight plan was filed for the 14 CFR Part 91 positioning flight. The airplane was substantially damaged. The airline transport-rated pilot and commercial-rated copilot reported no injuries. The flight had departed Anderson, North Carolina, en route to Charlotte at 0545. The pilot told the FAA, "...that he had positive gear down and locked indications and touched down at 85 knots. The airplane yawed right and nosed over, the aircraft was out of control at this point." Examination of the airplane by the FAA revealed that a right main wheel brake had locked up, and the landing gear had collapsed. According to the FAA inspector's statement, "...a pronounced and uniform skid mark from the point of initial brake lockup to a point approximately 2,500 feet from the landing threshold was evident. After exiting the runway, the right main landing gear collapsed." Inspection of the right main landing gear assembly and all associated components could not provide any insight as to what caused the main wheel brake to lockup.
