M/R BLADE STRUCK 3 ACTORS ON GROUND. THE BOARD DETERMINED THAT THE PROBABLE CAUSE OF THE ACCIDENT WAS THE DETONATION OF A DEBRIS-LADEN SPECIAL EFFECTS EXPLOSION TOO NEAR THE ACFT.
