About

Aviation safety reports, made easier to study

NTSB Reports is an independent research and search tool for public aviation accident and incident records. It brings NTSB aviation reports, structured fields, docket files, extracted figures, photos, videos, and update history into one searchable interface.

The goal is practical safety education: make it faster to find relevant investigations, compare similar events, review factual records, and understand the evidence behind probable-cause and preliminary reports.

The investigative work and source records belong to the National Transportation Safety Board and the published investigation record. This site is not affiliated with or endorsed by the NTSB.

The pilot stated the loss of engine power was due to fuel exhaustion.

NTSB Probable Cause

The pilot’s improper fuel management, which resulted in a total loss of engine power due to fuel exhaustion.

Full Narrative

The pilot told FAA inspectors that he was returning from the Reno Air Races and had stopped in Flagstaff, Arizona, to refuel. On final approach to Hicks Airfield, the engine lost power, and the pilot made a forced landing on rough terrain. Both upper and lower left wings were crushed. The pilot said that the engine lost power due to fuel exhaustion. On several occasions, the pilot was requested to complete and return NTSB Form 6120.1-2, Pilot-Operator Aircraft Accident Report. He failed to comply.
