On April 11, 2011, about 1350 coordinated universal time, a Bell 206B-III, 9M-AXH, collided with terrain during takeoff near Sibu, Malaysia. The pilot was fatally injured and the helicopter was substantially damaged. The helicopter was operated by Sabah Air Aviation under the pertinent civil regulations of Malaysia.
