On March 16, 2011, about 0718 coordinated universal time, a Bell 412EP helicopter, United Arab Emirates (UAE) registration A6-FLV, experienced a failure of the landing gear aft cross tube during loading of the helicopter at Zakum Field, UAE. The investigation is under the direction of the General Civil Aviation Authority (GCAA) of the UAE. As the state of design of the helicopter, the NTSB has been requested to assist the GCAA in their investigation.
