On January 19, 2014, at 1220 local time, a Piper PA-31-350, PK-IWT, collided with terrain under unknown circumstances while on approach to Dumatubun Airport, Maluku, Indonesia. The pilot and three passengers were fatally injured and the airplane was destroyed. The airplane was registered to and operated by Intan Angkasa Air Service under the pertinent civil regulations of the government of Indonesia.
