On April 9, 2014, at 0025 universal coordinated time, a Quest Aircraft Company, Kodiak-100, PK-SDF, during takeoff from Doyo Baru Airstrip, Jayapura, Papua, Indonesia, the airplane lost altitude and struck the airport fence and subsequently came to rest on a roadway. The airplane was operated by Adventist Aviation, under the pertinent civil regulations of the government of Indonesia. The pilot and one passenger were fatally injured. Two passengers were seriously injured and three passengers and two people on the ground received minor injuries. The airplane was substantially damaged.
