On March 21, 2015, about 1030 local time, a Sikorsky S-76C+ helicopter, registration PK-FUP, sustained substantial damage following an emergency landing near East Kalimantan, Indonesia. Reports indicate that the two pilots received serious injuries and five passengers received minor injuries. The pilot's planned destination was an offshore oil platform. The accident is being investigated by the National Transportation Safety Committee (NTSC) of Indonesia. In accordance with ICAO Annex 13, the NTSB has appointed a U.S. Accredited Representative as the State of Manufacture and Design.
