On June 28, 2016, about 1350 Universal Coordinated Time (UTC), a Vans RV6, C-TFUO, impacted terrain at St-Mathieu-De-Beloeil (CSB3), Quebec, Canada. The pilot, the sole occupant on board, was fatally injured. The airplane was substantially destroyed. The airplane was registered to and operated by a private individual as a personal flight. Visual meteorological conditions prevailed at the time of the accident, and no flight plan had been filed. The local flight was originating at the time of the accident. Canadian officials said the airplane took off from Runway 15 for a local flight in the area. On initial climb, a loud bang was heard and the airplane was observed in a descent. The airplane collided with level terrain in a field approximately 1000 feet from the end of the runway before coming to a stop 220 feet beyond the initial impact. A post impact fire ensued. The accident is under the jurisdiction and control of the Canadian government.
