On December 7, 2016, about 1150 UTC, an Industria Aeronautica Neiva S/A EMB 721D, Brazilian registration PT-REI, registered to and operated by a foreign individual, collided with trees then terrain shortly after takeoff. Visual meteorological conditions prevailed at the time and flight plan information is unknown for the non-U.S., non-commercial flight that originated 1 minute earlier from Manaus/Flores Airport, Manaus, Brazil, destined for Novo Aripuana Airport, Aripuana, Brazil. The airplane was destroyed, and the commercial pilot and 5 passengers were fatally injured. Shortly after takeoff from runway 11, witnesses reported the engine experienced a loss of power and the airplane was observed losing altitude. During a forced landing in a wooded area east of the airport, the airplane impacted trees then came to rest inverted on the ground. One passenger survived, but died while hospitalized shortly after the accident. The investigation is under the jurisdiction of the Government of Brazil.
