On June 25, 2017, at 1200 Coordinated Universal Time, a Piper L4, OM-M038, impacted terrain after takeoff from Hodkovice Airport (LKHD), Liberecký, Czech Republic. The airplane was destroyed. The pilot sustained serious injuries, and the passenger sustained fatal injuries. The airplane was registered to and operated by the pilot. The flight was originating at the time of the accident and was destined to Plasy Airport, Plzenský, Czech Republic. The investigation is under the jurisdiction of the Czech Republic government.
