On June 25, 2017, at 1200 Coordinated Universal Time, a Piper L4, OM-M038, impacted terrain after takeoff from Hodkovice Airport (LKHD), Liberecký, Czech Republic. The airplane was destroyed. The pilot sustained serious injuries, and the passenger sustained fatal injuries. The airplane was registered to and operated by the pilot.
