Aviation safety reports, made easier to study

NTSB Reports is an independent research and search tool for public aviation accident and incident records. It brings NTSB aviation reports, structured fields, docket files, extracted figures, photos, videos, and update history into one searchable interface.

The goal is practical safety education: make it faster to find relevant investigations, compare similar events, review factual records, and understand the evidence behind probable-cause and preliminary reports.

The investigative work and source records belong to the National Transportation Safety Board and the published investigation record. This site is not affiliated with or endorsed by the NTSB.

According to the operator, Guardian Flight LLC, dba Hawaii Life Flight, the medical transport flight was en route, with two medical personnel on board from the Kapiolani Medical Center, to pick up a patient on a neighboring island. According to the pilots, just after starting the engines, the airplane began an un-commanded roll forward and it subsequently struck an unoccupied, parked company Raytheon Aircraft Company C90A airplane, N902TS, which was located across the ramp. As a result, N438CA sustained substantial damage to the right engine firewall and engine mounts. The wreckage has been moved to a secure location pending further examination.
