About

Aviation safety reports, made easier to study

NTSB Reports is an independent research and search tool for public aviation accident and incident records. It brings NTSB aviation reports, structured fields, docket files, extracted figures, photos, videos, and update history into one searchable interface.

The goal is practical safety education: make it faster to find relevant investigations, compare similar events, review factual records, and understand the evidence behind probable-cause and preliminary reports.

The investigative work and source records belong to the National Transportation Safety Board and the published investigation record. This site is not affiliated with or endorsed by the NTSB.

Full Narrative

On May 4, 1993, at 0900 central daylight time, a Rockwell S-2R, N424OX, was substantially damaged while maneuvering near Lookeba, Oklahoma. The commercial pilot, also the owner/operator, sustained minor injuries. Visual meteorological conditions prevailed. During an interview with the pilot, conducted by a Federal Aviation Administration (FAA) inspector, he stated that he had completed a spray run and was maneuvering for the next pass when the cockpit filled with smoke and the engine lost power. He further reported that after he lost engine power his right wing struck a telephone pole and he descended uncontrollably to ground impact. The operator was unable to determine why the engine lost power. As a result of a bankruptcy proceeding the was removed from the warehouse and could not be located for additional inspection.
